How Telehealth Ketamine Therapy Works

Telehealth ketamine therapy is one of the most significant access innovations in modern mental health care — but the process is opaque to most people considering it. This article walks you through every step, from your first inquiry to ongoing maintenance, exactly as it works at NutraBrain Clinic.
Step 1 — Initial eligibility assessment
The process begins with a brief assessment. At NutraBrain that’s a 20-second qualifying quiz on our site. The purpose is to screen for conditions that would make at-home ketamine inappropriate (active psychosis, uncontrolled bipolar disorder, severe cardiovascular disease, current pregnancy, severe substance use disorder without stable recovery). This isn’t a marketing tool — patients who don’t pass this screen are not enrolled.
Step 2 — Intake form
A structured form covering full medical history, current medications (especially MAOIs, opioids, benzodiazepines, anti-seizure drugs), mental health history and prior treatments, current symptoms and severity, treatment goals, home environment safety, and emergency contact information. Typically takes 10–15 minutes.
Step 3 — Video consultation with Dr. Mahjoubi
At NutraBrain this step is non-negotiable: every patient meets with me directly via video before being prescribed. This is not standard across the telehealth ketamine industry — many programs use rotating nurse practitioners — but it’s how the program was designed. The consultation takes about 30 minutes. Step 4 — Prescription and pharmacy fulfillment
Once we agree on a protocol, I send your prescription to one of three U.S.-licensed compounding pharmacies: Novel Compounding Pharmacy (Sylmar, California), Apothicare 360 (Lee County, Florida), or The Chemist Shop (Long Island City, New York). These are 503A compounding pharmacies under state board of pharmacy oversight. Every batch carries a Certificate of Analysis verifying potency and purity. The pharmacy fills your prescription within 1–3 business days and ships directly to your home via signature-required courier.
Step 5 — Pre-session preparation
Before your first session you’ll receive a written protocol covering your specific dose and how to administer it, time of day (typically late afternoon or early evening), what to eat and drink (light meal 2–3 hours prior, no alcohol), what to have ready (eye mask, headphones, blanket, water, notebook), who should be in the home (a sober adult should be aware and reachable), and what to expect minute-by-minute. Pre-session ritual matters.
Step 6 — Your first session
Typical timeline: 0 minutes — administer prescribed dose. 5–15 minutes — first effects emerge, gentle floating sensation, increased openness, mild dissociation. 15–45 minutes — peak therapeutic window where most of the antidepressant, anxiolytic, and trauma-processing work occurs. 45–75 minutes — wind-down, gradual return. 75+ minutes — return to baseline; most patients sleep deeply that night.
Do not drive, operate machinery, make important decisions, or care for children for the rest of the day after a session.
Step 7 — Post-session integration
The first 24–72 hours after a session is where much of the benefit consolidates. Journal what came up. Limit decisions and screen time the next morning. Notice mood, sleep, energy, and changes in thought patterns. Email observations or concerns to me directly. Many patients pair sessions with ongoing psychotherapy — the two work together because ketamine creates a window of neuroplasticity, and therapy helps you do useful work within it.
Step 8 — Ongoing protocol and refills
After the first month, treatment shifts into an ongoing protocol designed around your response. Continued sessions at a frequency fitting your goals (often weekly or biweekly initially, then less frequent). Refills shipped automatically based on usage.
